VacationGuard Assist is a 24/7 benefit provided free of charge to active enrolled individuals in
the VacationGuard Assist program. Enrollment is limited to companies who qualify under the VacationGuard plan parameters which are extended to the participating company, association, or qualified participants. This benefit applies when the enrolled participant is traveling over 100 miles from home, on either business or pleasure. This is a general overview and not meant to outline all benefits, services, or exclusions. Read the complete Description of Covered Services within the member section for a full explanation. There is no coverage unless you are enrolled and have a certificate number assigned.

MEDICAL TRANSPORTATION SERVICES:
• Emergency Evacuation: If You or Your dependent suffers Injury or Sickness and adequate medical facilities are not available locally, VacationGuard Assist will arrange and provide emergency evacuation to the nearest facility capable of providing adequate care.
• Medically Necessary Repatriation: After initial treatment and stabilization for an Injury or Sickness, if deemed medically necessary, VacationGuard Assist will arrange and transport You back to Your permanent place of residence for further medical treatment.
• Repatriation of Mortal Remains: In the event of death, VacationGuard Assist will render assistance and provide for the return of mortal remains and making travel arrangements for any traveling companions.
• Visit by Family Member or Friend: If You are hospitalized for more than seven (7) days and are traveling alone, VacationGuard Assist will arrange and provide Your family member or friend with transportation to visit you in the hospital.
• Return of Dependent Children: If You are hospitalized for more than seven (7) days, VacationGuard Assist will arrange and provide the return of Your minor children who are under eighteen (18), with a qualified attendant.
• Traveling Companion Transportation: If Your travel companion loses previously made travel arrangements due to Your medical emergency, VacationGuard Assist will arrange and pay for Your traveling companion's return home.

| QUALIFICATION FOR ENROLLMENTS: |
1. Only companies which meet and maintain the standards as determined solely by VacationGuard can be submitted to obtain these year-round VIP benefits.
2. Your company may have up to 15 covered individuals, who must be an actively employed Manager, Officer, Supervisor, Current Association Board Member, or otherwise as agreed to with VacationGuard.* Only 2 individuals will be able to add and edit the covered members for your company.
3. Travel Assist coverage is effective for the member on the 1st of the next month following the date of their on-line enrollment activation. (ie: enroll by 4/25 to get coverage for 5/1)
